In September 2022, the dynamic duo of James and Alexa founded Chicago Pet Club, inspired by a chance meeting when James offered to walk Alexa's dog in their shared building. Recognizing a gap in pet care services in the Lakeview/Uptown area, they seized the opportunity to create a business that would not only meet the needs of local pet owners but also foster a sense of community.

James, hailing from a family farm in Kansas, brought a lifetime of experience with pets to the venture. His deep understanding of animal care, coupled with a genuine love for creatures big and small, became the cornerstone of Chicago Pet Club's commitment to excellence. Alexa, with a background in education and a passion for pets, complimented the team with her organizational skills and dedication to creating a welcoming space for pet owners.

The journey began with just a handful of clients, but the quality of their service and the genuine care they provided quickly earned Chicago Pet Club a stellar reputation. In no time, the business flourished, boasting a clientele of over 500 satisfied customers. With over 35 years of combined pet care experience, James and Alexa built a team that shared their enthusiasm for animals and commitment to exceptional service.

Client Agreement 

 

BY SIGNING, in consideration of the services rendered by Chicago Pet Club to my pets, I acknowledge reading, understanding, and accepting the statements herein. AGREEMENT TO PARTICIPATE AND LIABILITY WAIVER I understand certain “activities” that my dog/pet may participate in, including daycare, boarding, one-on-one play, walks, grooming, training, movement within and outside the facility, and transportation to and from Chicago Pet Club involve risk and possible injury, to me, my companions and/or my dog/pet including but not limited to: Exposure to parasites, viruses, and other medical conditions passed from dog-to-dog or person-to-dog;  Sprains, strains, bites, broken bones; Motor vehicle accident during transportation; and  Fatigue, dehydration, nicks, cuts, or death. I further understand that not each and every potential risk can be listed above but, nonetheless agree that the benefits associated with dog socialization outweigh the possible risks, therefore, I hereby freely and voluntarily release, and forever discharge Chicago Pet Club and its agents, successors, heirs and assigns from any and all liability, claims, demands, actions, or rights of action, which are directly or indirectly related to, arising out of, or are in any way connected with my dog’s participation in activities at Chicago Pet Club, including those allegedly attributable to the negligent acts or omissions of Chicago Pet Club or their staff. Further, I understand that I may be exposed to certain risks when bringing my pet to participate in activities at Chicago Pet Club or when picking up my pet from participating in activities at Chicago Pet Club. Such risks may include property damage and/or physical injury inside or outside the facility, such as from falling, slipping, illness, and/ or dog bites. Therefore, I hereby voluntarily release, and forever discharge Chicago Pet Club, its agents, successors, heirs, and assigns from any and all liability, claims, demands, actions, or rights of action, related to, or which may arise out of, or are in any way directly or indirectly connected with my pet’s participation in activities at Chicago Pet Club, including those allegedly attributable to the negligent acts or omissions of Chicago Pet Club or their staff. 

 

AUTHORIZATION OF MEDICAL CARE: If my dog is ill or injured while participating in activities at Chicago Pet Club, Chicago Pet Club will make every reasonable effort to reach me pursuant to the contact information I have provided Chicago Pet Club. However, if Chicago Pet Club is unable to reach me, I consent to Chicago Pet Club seeking appropriate veterinary care and I accept responsibility for any and all associated expenses. Chicago Pet Club will not pay any portion of veterinary expenses associated with seeking medical care for my dog/pet in the latter instance. 

 

ALLERGIES, SPECIAL DIETS, MEDICATIONS: I agree that I will disclose to Chicago Pet Club any allergies my dog may have. I further agree to disclose to Chicago Pet Club any special dietary needs or medications my dog may require if necessary during activities at Chicago Pet Club. Please list any allergies, special diets, and medications in your pet's profile in our Time To Pet System. 

 

VICIOUS TENDENCIES: I affirm that I am not aware of any vicious tendencies by my Dog/Pet.

 

AGREEMENT TO PAY: Chicago Pet Club accepts cash or credit cards. I agree to pay the service rates in effect for my dog’s/pet’s participation in activities at Chicago Pet Club. I further agree to pay for any additional services requested such as grooming, after-hours, weekend rates, cleaning fees, damage fees, drop-off, and/or pick-up service, etc. All pricing is subject to change. All sales are final and no refunds. Pricing is listed on the website and portal.

 

DAMAGE: I accept the responsibility of paying for any damage to the facility, property, and/or equipment caused by the Dog.

 

VETERINARY RECORDS: My dog’s/pet’s complete veterinary records must be furnished to Chicago Pet Club. These records must include proof of vaccinations and/or treatment for: parvovirus, distemper, Bordetella, heartworm, fleas, and ticks. Records must be submitted into the system before a service can be booked. I further attest that my dog is free of parasites and other illnesses that can be transmitted from dog to dog. Due to the high risk of dog-to-dog transmission of such parasites and/or viruses, I agree that I will immediately notify Chicago Pet Club if I learn or suspect my dog has parasites or viruses and agree to not bring my dog to Chicago Pet Club for any activities until I receive clearance from Chicago Pet Club, in conjunction with my dog’s veterinarian.

 

EVALUATION OF DOG PRIOR TO PARTICIPATION: Every dog must be evaluated by Chicago Pet Club prior to participating in any activity, grooming, or training activities. Such evaluation may assess the dog’s temperament and interactions with other dogs and Chicago Pet Club

staff at the sole and exclusive discretion of Chicago Pet Club.

 

MEET & GREETS: Every new pet must complete a “Meet & Greet” that can be booked through the app before any services can be confirmed. Key hand-offs are the responsibility of the owner and will need to be dropped off to a member of Chicago Pet Club during the "Meet & Greet" or at a scheduled time.

 

PHOTOGRAPHS: This release authorizes Chicago Pet Club to take photos of your pet for client files and for the company website and Facebook page. All photos taken are at the property of Chicago Pet Club or during walks/visits. 

 

RIGHT TO DECLINE: I understand that Chicago Pet Club reserves the exclusive right to decline participation or to terminate participation in activities at Chicago Pet Club to any dog/pet at any time for any reason. 

 

CANCELLATIONS: Under the "Schedule" section of the portal or app, you can make a request for service and even cancel free of charge dependent on the service up to 72 - 24 hours in advance if not you will be charged 20% of the booked service.

 

COVID-19 POLICY: If you are sick or feel sick you must cancel your services free of charge but you must make us aware as this may put us at risk or in harm's way. For an additional $10 service fee we will continue with services but your pet must be leashed up and ready to go before we get there and the client must have a mask on. 

 

GROOMING ACCIDENTS: Grooming equipment is sharp, and although we use extreme caution and care in all situations, possible problems could occur including cuts, nicks, scratches, quicking of nails, etc. Your pet’s safety and comfort are our number one priority. In the event an accident does occur, you will be notified of the accident. If Chicago Pet Club feels it is serious, and the owner is not on-site, and cannot be reached, Chicago Pet Club will seek immediate veterinary care for your pet. 

 

GROOMING HEALTH or MEDICAL PROBLEMS: Occasionally, grooming can expose a hidden medical problem or aggravate a current one. This can occur during or after grooming. All medical expenses for veterinary care will be covered by the animal's owner upon signing this agreement. 

 

GROOMING DANGEROUS or AGGRESSIVE ANIMALS-REFUSAL of SERVICE: Chicago Pet Club has the right to refuse any services at any time. In the event that your animal is too stressed or becomes dangerous to groom, Chicago Pet Club has the right to refuse grooming services, stop grooming services, or cancel grooming services at any time before, during, or after grooming and the client will be charged a grooming fee (for what was done up until that point). 

 

GROOMING USE of MUZZLES: Muzzling does not harm your animal and protects both the animal and the groomer. In some cases, muzzling may even calm a stressed animal, allowing the grooming process to continue. If an animal still acts in a way that is dangerous, Chicago Pet Club has the right to stop grooming services at any time and a service fee will be collected. We do not muzzle unless your pet gives us a reason to. Other methods are used to calm your pet, muzzling is a last resort. 

 

GROOMING MATTED COATS: Pets with severely matted coats require extra attention. Mats in a pet’s coat grow tight, and can ultimately damage and tear the pet's skin, which provides a breeding ground for parasite infestations. Chicago Pet Club will not cause serious or undue stress to your pet by de-matting. Mats can be very difficult to remove and may require the pet to be shaved. Removing a heavily matted coat can cause nicks, cuts, or abrasions due to skin growths trapped in the mats. Heavy matting can also trap moisture and urine near the pet's skin allowing mold, fungus, or bacteria to grow, producing skin irritations that existed prior to the grooming process. After-effects of mat removal procedures can include itchiness, skin redness, self-inflicted irritations or abrasions, and failure of the hair to regrow. In some cases, pets may also exhibit brief behavioral changes. Prevention is the best defense by scheduling regular grooming appointments, every 4-6 weeks. If your pet needs to be shaved to remove matting, by signing below, you acknowledge that you agree to this procedure and any risk. There will be an additional charge for this process: it is time-consuming, and causes extra wear and tear on grooming equipment. The additional charge can be estimated by the groomer prior to the appointment. 

 

GROOMING SATISFACTION: Your satisfaction is important to us. If you are unhappy for any reason and would like something adjusted, we will be happy to make any adjustments when you pick up your pet from his/her appointment. Once, however, you take your pet home from the appointment, any return visits will be treated as a new appointment and the client will be charged a grooming fee. 

ATTORNEY FEES, APPLICABLE LAW & VENUE: Should Chicago Pet Club, its agents, successors and assigns, or anyone acting on their behalf, be required for any reason to incur attorney fees and costs to enforce or defend this agreement, I agree to indemnify and reimburse Chicago Pet Club for such fees and costs provided that the same shall be fair and reasonable. Further, I agree and understand that any disputes arising out of this Agreement will be decided pursuant to the laws of the State of Illinois and the venue shall be in Cook County.

 

VALID DATES: These agreements, waivers, and authorizations will remain valid and in force as long as and whenever my dog participates in any activity at or with Chicago Pet Club. 

 

WARNING: By signing this document, I acknowledge that if my dog is or I am hurt or property is damaged during my dog’s/pet’s participation activities at Chicago Pet Club, I may be found, by a court of law, to have waived my right to maintain a lawsuit against Chicago Pet Club on the basis of any claim from which I have released them herein. I have had sufficient opportunity to read and fully understand this entire document and I agree to be legally bound by its terms.

We’ll maintain a record of your information at the offices of Chicago Pet Club, LLC so we can provide you with membership information. The employees of Chicago Pet Club, LLC are each required to sign and acknowledge a Confidentiality And Nondisclosure Agreement. Each employee has been instructed on maintaining the privacy of each customer and the importance of protecting the customer’s personal information.

When you become a member of a Chicago Pet Club, you’ll pay the membership fee with your credit card or personal check. If you use your credit card to pay the fee, your credit card information will be deleted from our website after your purchase has been authorized.

When you contact us from this website, you’ll provide us with certain personally identifiable information, including your e-mail address.

A cookie is a piece of data that’s stored on a visitor’s hard drive while they’re visiting this website. We may utilize a short-lived form of cookie when you visit this site.
